Patient Safety Lives in the Gaps Between Teams

Many patient safety failures belong to the space between departments.

Between the emergency department and the inpatient unit.

Between nursing and pharmacy.

Between the hospital and the next place a patient receives care.

Each team may complete its part correctly while the patient still experiences a gap.

That is why patient safety depends on more than strong departments. It depends on what happens when information and responsibility move between them.

A safe handoff should make four things clear:

  • What remains unresolved?

  • What could go wrong?

  • Who owns the next step?

  • How will we know it was completed?

In my newest Off the Clinical Path article, I explore why leaders must examine the spaces between teams—not only the performance within them.

What transition in your organization depends too heavily on memory, persistence or knowing the right person?

That may be a gap worth closing.

When Quality Metrics Become the Work

Every metric eventually becomes part of someone’s workday.

Healthcare needs measurement. We need to know whether care is safe, outcomes are improving and our efforts are making a difference.

But every new measure also creates work.

Another required field.
Another audit.
Another dashboard.
Another report.

Individually, each request may seem reasonable. Collectively, they can consume time that clinicians and leaders could be using to improve care.

A measure should help us see the work more clearly. It should not become the work itself.

In my newest Off the Clinical Path article, I explore how leaders can distinguish meaningful measurement from measurement that creates more burden than value.

Before adding—or continuing—a metric, ask:

What meaningful action does this measure drive?

If the answer is unclear, it may be time to reconsider what we are asking teams to produce.
